It’s killing your organization.

Understand the problem first.  Interview people.  Ask the right people the right questions.  Why?  And why is that? Get the right people together – managers, influencers and the people involved. Don’t seek to assign blame.  Lay it out.  Call it out.  Take responsibility.  Show you care.  Paint a clear picture of a better future.  Ask for their help.  Then take any specific action necessary.

Well said. One of the marks of a good leader is their ability to very clearly identify the goals and purpose of the enterprise, focus the entire organization on achieving the goal and purpose, and get everyone pulling in the same direction to achieve those results.
