Have you created bureaucracy in your business as a knee jerk reaction to try to avoid mistakes?

Any dummy can ADD a step, a form, a checklist, or an approval process.  Keep doing that and your organization has transcended the problem by becoming the problem.

Stop the madness.  Blow it up.
